FAQ

Do I need a referral from my doctor to be seen?

No. In the state of Iowa, you do not need a referral from your doctor to be seen by a physical therapist. If you have a referral, please make it available at your first appointment. I will fax any pertinent notes regarding your treatment plan to your doctor. Collaboration with other providers on your healthcare team is always encouraged.

Can I use my insurance?

Hatch Physical Therapy accepts Blue Cross Blue Shield insurance plans for office visits. Hatch Physical Therapy is an out-of-network provider for all other insurance plans.  If you would like to submit it to your insurance company personally for reimbursement, I will provide you with a superbill after each visit. I will be considered “out of network”.  Reimbursement for out-of-network services is not guaranteed.

Can I use my HSA or FSA account?

Yes. Acceptable forms of payment include HSA/FSA accounts, check, cash, or credit card.

Do you offer home visits?

Yes. Home visits are offered on request, but weekly availability for home visits is limited. There is a $20 mileage fee for time and travel costs. 

What should I wear to my appointment?

For women and children, please wear clothing that is comfortable and easy to move in. If I am assessing your baby, I may ask you to dress them down to their diaper during my assessment.
